Back to School Info & Forms

Below, is information needed to get started for the new school year. A mailing was sent to your home the week of  August 1, 2016 asking you to print and complete school forms and update your on-line data through PowerSchool.  All required forms are posted here for your convenience under the FORMS section below.  Forms are due no later than August 29 -- the first day of school.  Please complete all forms and mail them back to the school OR...Bring your completed forms with you to Sneak-A-Peek Day, August 26, from 1:00-3:00 pm.  You can drop your forms off in the gym.

CLICK HERE to access our Forms Management Page -- this is where you will find all the forms required by the school.  You will be asked to read, print, date and sign forms, returning them to school by August 29, the first day of school.  Additionally, you are required to review and update information via your PowerSchool Parent Portal this year. Thank You! 

SNEAK-A-PEEK DAY -- AUG. 26, 1:00-3:00PM

“Sneak- a- Peek” Day: August 26, 1:00 – 3:00 pm

Parents and students in grades 1-8 are invited to an informal meet and greet with teachers in classrooms. You are welcome to visit anytime during these hours for a brief visit. Students may bring their school supplies with them that day. 

*Kindergarten students will have their Meet and Greet on Thursday, August 25.  Additional information will be sent in a letter to each kindergarten family in August.

Educational Outfitters – our school PE uniform supplier will be present to answer questions and take
orders. Some items may be for sale at this time.

Completed Back to School Forms may be dropped off in the gym. All forms due first day of school, August 29.


School begins at:

7:55 am for grades 6, 7, and 8

8:00 am for grades PreK – 5

Students may begin to arrive at 7:30 AM.

Middle school students may enter Stephens Hall upon arrival (after 7:30 am).

Monday/Tuesday/Thursday and Friday: School will be dismissed at 3:15 pm.

Wednesday: School will be dismissed at 2:15 pm.

School Supply lists are available on this website under Parent Community. Each student should come prepared for the first day of school with the supplies requested.

We will continue to use Edline, a password protected link, as part of our communication system. All new parents will receive instructions for signing on with an initial user name and password before the start of school (in their summer mailing.) If you have not received this information or are having difficulty signing on, please contact Lisa Lucrezi-Bond at after August 22. Returning students and parents may use their name and password from the prior school year.

Students in grades 4-8 can have their own student account on Edline.  Parents can request that their student's account be activated by contacting Lisa Lucrezi-Bond at



All prescription and over-the-counter medications must be brought in by the parent with the Medication Authorization form. All medications must be in the original container with the child’s name, doses to be given, and the time to give the medication.

Medication Authorization form will be necessary to provide your child with Tylenol or Motrin for the school year, for headache, orthodontia or muscular pain.

Inform the school nurse if your child is currently taking medication, or if your child has special medical considerations, such as: diabetes, asthma, ADHD, or specific allergies (food allergies, tree allergies, etc.) If your child requires medication for a chronic condition such as asthma, or for an allergic reaction, please print out the appropriate forms, and give to your child’s physician to complete. The appropriate form must be sent in with the medication, in order for the medication to be given to your child.The school nurse cannot take verbal orders to give these medications. Returning students must complete new forms each school year.

All parents must complete the Confidential Student Health History Update Form for each child.

Report to the nurse any sprains, broken bones, or upcoming surgeries your child may experience during the school year.

Do not send your child to school if he/she has had a fever of over 99.6 F degrees, has vomited or has diarrhea within 24 hours. This helps to prevent other children from contracting the illness. It is difficult for children to concentrate on school work when they do not feel well.

Ensure your child’s health and success in school by helping him/her get a good night’s sleep and a healthy breakfast each day.

The 2016-2017 PreK and Elementary School (K-8) Parent/Student Handbooks will be posted before the start of school.  Click on the pdfs near the right margin of this page to access the handbooks. After reading the handbook with your student, print and sign the Handbook Agreement Form (found in the Forms section above). Students in grades 6-8 must also sign the Handbook Agreement Form. Forms are due August 29.

Note, there is a separate Handbook Agreement Form for the parents of PreK students.  


Click here for EZschool Pay Sign up Instructions. Hot and cold lunch offerings are made available to our students. Please set up your EZ School Pay lunch account for your child(ren), if you do not have one.  All families are required to use EZ School Pay this year to pay lunch bills; please note that there is a fee of $10.00 each time the school has to bill the family directly. You can check to see if you are enrolled at

If you need your student ID to set up your accounts, please send your child(ren's) name(s) to Bridget Michael at  and she will email the ID numbers back to you.

Once your account is set up, or if you are a returning account holder, now is a good time to add credits to your account -- simply select Make A Payment, and you can add credit to your student's account.

Milk order forms are located on the Forms Page under Optional Forms. Please print this form and complete it if you would like your child(ren) to get milk.
