Virginia State Required Immunizations

Students entering Pre-K, Kindergarten, or transferring to The Basilica School of Saint Mary, must have a completed School Health Entrance Form, to include the child's physical and the minimum immunization record dated within one calendar year of the first day of school, in their health file. Students entering other grades should have a Confidential Health History Form completed with updated immunization records (if applicable) the first day of school. Please ask your physician for a copy to provide to the school health office.

Students entering the Seventh Grade

Tdap: Effective July 1, 2019, a booster dose of Tdap vaccine (Tetanus, diphtheria, and Pertussis) is required for students entering the 7th grade.

The Tetanus, diphtheria and pertussis (Tdap) booster has previously been required prior to entry to 6th grade. The recommended age for Tdap has been changed from 10 years of age to 11 years. THe Tdap booster will now be required prior to entry to 7th grade. Please note, the recent Tdap changes WILL NOT affect current rising 7th grade students.

Students entering the Sixth Grade

Human Papillomavirus Vaccine (HPV): Effective October 1, 2008, the HPV vaccine is required for females and recommended for males. The HPV is a two dose series of vaccines for students under the age of 15. This vaccine provides protection against cancers that are caused by HPV infections. Please consider the information provided prior to immunizing your child. The first dose shall be administered before the child enters the 6th grade.

Parents who choose not to immunize their child/children must complete a Certificate of Religious Exempt Form. This form must be notarized.
