Virginia State Requirements

Students entering Pre-K, Kindergarten, or transferring to The Basilica School of Saint Mary, must submit a completed School Health Entrance Form along with an up-to-date immunization record prior to the first day of school. Rising Kindergarten students are required to provide an updated School Health Entrance Form and immunization record even if they have attended Pre-K at The Basilica School of Saint Mary.

Students entering into 3rd and 7th grades are required to provide a School Heath Entrance Form along with an up-to-date immunization record. Students entering other grades are required to submit a Confidential Health History Form (yearly update) at the beginning of each academic year.

Health forms may be submitted to the school health clinic at any time. All forms need to be received prior to the first day of school.  Forms may be sent by mail to the office or submitted by email to

Students entering the Seventh Grade

Tdap: Effective July 1, 2019, a booster dose of Tdap vaccine (Tetanus, diphtheria, and Pertussis) is required for students entering the 7th grade, previously required for students entering the 6th grade. Proof of immunization must be provided to the school health office prior to the first day of school. Tdap VDH Fact Sheet

Students entering the Sixth Grade

Human Papillomavirus Vaccine (HPV) provides protection against cancers that are caused by the human papillomavirus. Effective October 1, 2008, the HPV vaccine is required for females and recommended for males. The HPV vaccine is a two-dose series of vaccines for students under the age of 15. The first dose is generally administered before the student enters the 6th grade. Please review the information provided.
